Hospital Report Card Act
The Illinois Department of Public Health (IDPH) is responsible for
making certain the adopted rules of the Hospital Report Card Act* are
followed. The Act was passed by the Illinois Legislature in 2006 and
became effective in March, 2007. Basically, the Act requires hospitals
to provide to patients and anyone inquiring the number of nursing staff
assigned to a patient unit during a work shift.
Nurse staffing includes the number of registered professional nurses,
licensed practical nurses and other nursing personnel assigned to each
patient unit each day.
For requests regarding nurse staffing levels while a patient is in the
hospital for treatment, ask to speak with the Acute Care Clinical
Coordinator, Nursing Supervisor or Nursing Director of the area in which
you are receiving care. For requests unrelated to an individual’s
pending or current inpatient hospital stay, persons should send written
requests for nurse staffing information to OSF Saint James Medical
Center, c/o Administration, 2500 West Reynolds Street, Pontiac, IL,
61764. Requests made in person or by telephone (815-842-2828, extension
3310) Monday through Friday between 9 a.m. and 5 p.m. will also be
accepted, however must be followed with a written request within seven
business days.
Current nursing staff schedules are available upon request at each
patient care unit, on forms that exclude confidential patient and staff
information.
