OSF Saint James Annual Golf Outing ~ June 8, 2009

OSF Saint James Medical Center is a State of Illinois-designated “resource hospital.” With that status, it is responsible for overseeing the recruitment, training and education of the area Emergency Medical Services system, including its 23 affiliates, with the needed personnel and all aspects of care delivered by them. OSF Saint James Foundation will be providing a large portion of the funding needed to continue training and program support through our annual golf outing. Our event is scheduled for June 8, 2009 at the Pontiac Elks Country Club.

People’s lives often depend on the quick reaction and competent care of emergency medical technicians, paramedics and other personnel. Help us to continue their training and education by supporting the annual golf outing through a corporate, business or hole sponsorship, a donation or participation in the event.

Win a New Car or Cash!
OSF Saint James Foundation is giving away a new vehicle or cash to benefit the Surgical Unit and surgical technology at OSF Saint James - John W. Albrecht Medical Center. Winner will have choice between a 2008 Dodge Avenger or 2008 Mercury Milan (must select one) or take $10,000 cash! The cars are from Heller Lincoln Mercury Dodge, Inc. of Pontiac, Illinois. The second prize will be $1,500 and three third prizes of $500.00 each will be awarded.

Tickets are $25.00 each, 3 for $50.00, 8 for $100.00 or 20 for $200.00.

Tickets will be mailed to you.

If you decide to mail in a check, please make it payable to OSF Saint James Foundation, 2500 West Reynolds Street, Pontiac, IL 61764-9774. We will complete the registration part of your ticket(s) with your name, address and telephone number. The ticket stub(s) will be mailed to you. Checks for the raffle must be received by OSF Saint James Foundation by September 18, 2008 by 12:00 noon.

Proceeds benefit the Surgical Unit and Surgical Technology at OSF Saint James Medical Center.

Drawing will be held on September 19, 2008, 12:00 noon in the Medical Center Lobby. Winner need not be present to claim prize.

2008 Car Raffle Rules
The OSF Saint James - John W. Albrecht Medial Center Car Raffle is conducted by the OSF Saint James Foundation, a not-for-profit corporation organized under the laws of the State of Illinois.

1. The purchase price of a 2008 Car Raffle ticket is 1 for $25.00, 3 for $50.00, 8 for $100.00 or 20 for $200.00. Cash, credit cards and money orders should be payable to OSF Saint James Foundation. Payments are not tax-deductible contributions. Purchase of tickets may be made at OSF Saint James Medical Center Foundation, Information Desk, OSF Medical Group offices at OSF Saint James Medical Center and OSF Medical Group Clinics, or click here to download a gift form. Tickets may also be purchased at the following businesses: OSF Saint James Medical Center Foundation, Information Desk at OSF Saint James and all OSFMG Medical Groups within the medical center. Tickets may also be purchased at OSFMG Clinics (Dwight, Chatsworth, Chenoa, Cornell, Fairbury, Flanagan, Minonk, Drs. If an entrant’s check is dishonored or credit card rejected by the bank, the entry will be declared invalid.
2. Completed ticket stubs must be delivered to and in the possession of OSF Saint James Foundation by 12:00 Noon, September 18, 2008 in order to be eligible for the 2008 Car Raffle.
3. Purchasers of raffle tickets must be 18 years of age or older and must qualify under Illinois law.
4. The prize shall be a choice of a 2008 Mercury Milan, 2008 Dodge Avenger or $10,000 cash.*
5. The name and photo of the winner will be posted in the OSF Saint James Medical Center Information Desk and OSF Saint James Foundation, as well as on this website.
6. The winner will be determined by a random drawing conducted by the Sisters at OSF Saint James - John W. Albrecht Medical Center, 2500 West Reynolds Street, Pontiac, Illinois. The drawing will be Friday, September 19, 2008 at 12:00 noon, in the medical center’s lobby. Winners will be notified in writing within seven (7) days.
7. The winner of the Car Raffle must make an election in writing between the 2008 Dodge Avenger or 2008 Mercury Milan or $10,000 cash.*
8. Winners will assume and be responsible for all local, state, and federal taxes, including without limitation all applicable sales, use, luxury, income, property and special taxes.
9. *If the cash equivalent of 2,000 tickets is not sold by September 12, 2008, OSF Saint James Foundation reserves the right to modify the Raffle and withdraw the prize of a car and award cash prizes based on the percentage of ticket sales.
10. More than one name may be stated on the registration form; however, the first listed name shall be deemed the “entrant.” The Social Security number of the entrant must be provided to claim a prize. Payment or delivery of any prize shall be made solely to the entrant. It is the sole responsibility of the entrant in the multiple party entry to allocate any such prize among the other participants in the multiple party.
11. The decision of the OSF Saint James Foundation shall be final on all matters concerning the raffle.
