To make a gift today by postal mail, please print out the gift form and mail it to OSF Saint James Foundation, 2500 W. Reynolds, Pontiac, IL 61764-9774 or fax your credit card gift to 815-842-4912.

Stock/Securities

To make a gift of securities today, please contact the Foundation office at 815-842-6827 to arrange a transfer.

Grain

To make a gift of grain, please contact the Foundation office at 815-842-6827 to arrange a transfer.

Real Estate

If you would like to invest in the future of OSF Saint James - John W. Albrecht Medical Center by donating real estate or designating specific property for OSF Saint James in your will, please contact the Foundation office at 815-842-6827.

Pay tribute to a loved one, mark a special occasion, a promotion, or say thanks to a special OSF Saint James staff member who cared for you or a loved one during an illness or injury.

In Celebration of ...

Ask your family and friends to make a donation to the OSF Saint James Foundation in your honor for occasions like birthdays, anniversaries or holidays. These celebratory gifts can be designated to any program or service at OSF Saint James Medical Center. The Foundation will acknowledge your tribute gift by sending a special letter to the person you have chosen to honor with your gift. The amount of your gift will not be mentioned. A separate receipt will be sent to you.

Memorial Gifts

Express your sympathy by making a donation in memory of a family member or friend Memorial gifts are a lasting remembrance of a special person who has touched you during his or her life. OSF Saint James Foundation will acknowledge your gift by sending an appropriate note to the family or friend in whose name the gift was made. The amount of gift will not be mentioned. A separate receipt will be sent to you.
